Request for MCQ exam template

Please fill in the form below to request a Multiple Choice Questionnaire (MCQ) exam template.

Send one request for each exam, ie. each different set of questions (except for the same exam with different versions).

After a few working days (to process the data entered in the form), you will receive a PDF file (see an example): this is the file to print for the exam.

Several options are available for the formatting of the template and the grading: you can ask in the comment below.

After the exam, please return all the sheets (classed by version, if there are several versions) AND one containing the right answers for each version (ID: 00000001 for 'Version A', ID: 00000002 for 'Version B'). After the processing of data, you will get a PDF with the global results and a spreadsheet with details.

Do not staple anything to the sheets (they will be scanned).
