Request for MCQ exam template

You can fill in the form below to request a Multiple Choice Questionnaire (MCQ) exam template.

Please enter one request for each exam, ie. each different set of questions (except for the same exam with different versions).

After a few working days (to process the data entered in the form), you will receive a PDF file (see example): this is the file to print for the exam.

After the exam, please return all the sheets AND one for each version with the right answers (ID: 00000001 for 'Version A', ID: 00000002 for 'Version B'). After the processing of data, you will get a PDF with the global results and a spreadsheet with details.
